  5. HL7 activities with DICOM - Helmut Koenig (and Harry Solomon) submits the HL7 Imaging Integration / DICOM WG 20 Report January 2011:
    • DICOM Supplement 135 (SR Diagnostic Imaging Report Transformation Guide): The supplement has been approved final text by DICOM WG6 in August 2010. The document specifies the transformation of DICOM SR Basic Diagnostic Imaging Reports to CDA R2 and complements the successfully balloted HL7 CDA Diagnostic Imaging Report Implementation Guide.
    • DICOM Supplement 150 (Radiation Dose Summary Information in Radiology Reports): WG20/II is currently preparing the document for DICOM letter ballot. Feedback received in the public comment period was discussed at the October work group meeting in Boston.

      • - Procedure Note Implementation Guide: II/WG20 cooperated with the HL7 Structured Documents WG on this new implementation guide to make sure that the documentation of image-guided interventions is harmonized with the existing CDA Diagnostic Imaging Report (DIR) IG. The document was balloted through HL7 and has been approved as DSTU.
      • - II/WG20 is a co-sponsor of the HL7 CDA IG consolidation project and will cooperate with Structured Documents in order to harmonize existing implementation guides.

    • Keith Boone reports: Due to flight delays, I will not be at the HL7 Working Group meeting on Sunday to report on IHE activities using HL7 standards. What follows below is the brief summary I had planned on presenting.

        1. Document Encryption … how to exchange CDA content in encrypted form.
        2. XDS Link/Unlink Support using HL7 Version 2 Link/Unlink ADT messages

      • Patient Care Coordination: Developing Version 3 Care Record and CDA based templates for reporting results of problem/allergy/medication reconciliation processes.

      NCPDP – HL7 Activities
      • The Structure Product Labeling Activities Task Group tracks the activities of the SPL, offers suggestions to improve access and usability of the FDA Structured Product Label and Electronic Drug Listings, and monitors the work of the Guiding Coalition for feedback to the WG. This task group met during the WG meeting. They have completed and sent three letters (Inactive Ingredients, Medication Guides/Copyrights, and Overfills) on SPL recommendations to the FDA. One additional recommendation letter regarding the definition of “kits” is in draft and awaiting further research. Four issues have been identified and will be taken to the HL7 SPL Leadership Team/SPL Working Group for discussion. The task group identified more issues that will be discussed on future conference calls of the task group.
      • NCPDP/HL7 Eprescribing Functional Profile Task Group is monitoring the ballots of NCPDP and HL7 on the pharmacist/pharmacy interface functional profile and the standalone profile. See Ballot WG110046 Functional Profile.
      • The EHR/HL7 Task Group has focused on the use of the Structured SIG in the LTCPAC settings. Several examples have been reviewed and feedback has been provided to the WG10 Structured and Codified Sig Task Group. The Task Group also looked at the additional guidance provided by the Drug Enforcement Agency (DEA) for nurse as an agent and have identified additional questions to submit for clarification.

    • Chuck Jaffe or John Quinn
      • John defers to Mead Walker on the ICSR/ Adverse Event Reporting project with JIC in joint ISO ballot. This balloted May-September in HL7 and ISO. Comments have been resolved from that process and are nearly ready to resubmit the package to ISO for FDIS and special ballot for HL7. FDIS ballot stage implies no substantial changes. Comments from the last round did not speak to the structure but more about proper use of the Common Product Model as with the IDMP project. This has been a long process and been a challenge to provide material in formats agreeable to both HL7 and ISO. The US FDA as primary sponsor has struggled to keep their heads in the game over the length of the ballot process.
      • Tim Buxton next spoke on the IDMP projects. Project has links through common product model on ICSR. Another jcomponent is the substances effort. 5 standardes in enquiry stage since September 2010. 23rd is closing date and third co-terminating one month ballot in HL7 and another short ballot in CDISC before all is done. Parallel testing has been in progress and comments rolling back into the ballot process. Mead asks what wil the IDMP ballot look like, what form it will take. Tim response that it will be in the form of the ISO documents iteselves.
      • Garry on EHR –S FM 1.1 ISO/HL7 ballot 10871. Final version of R1 in November 2009. Activity is focused on R2 in the EHR work group. R1.1 is expected to complete balloting summer 2011. R2 should become a formal work item in March 2011 and participation is welcomed.
      • Grahame next spoke on the R2 Datatypes, closing Tuesday for FDIS. Comments will be resolved by ISO Secretariat and then publication can take place. Once that’s done he can kick off R3.
      • Heather Grain presents the SKMT /Glossary update. Andy Stechishin is thanked for the process to automate extracting the V3 glossary and importing it into SKMT. 2890 terms have been loaded, with over 3000 definitions – 39 terms have more than 5 definitions. It’s not all managed, but the information has been collected. Harmonization and Process for JIC was discussed for resolution of multiple definition harmonization. The intent is for one term has one definition in each context or language, to be used by the whole health informatics standards international community. The objective is to make our own lives easier. You do not have to be a member of one of the SDOs to use the web site but you do have to be a member to make changes. Process outline was reviewed. A demonstration was made at www.skmtglossary.org, using the term search for “class”. Heather further offers to provide more education to groups that want to learn more about how this works. The sooner we get everyone participating then the fewer harmonizations we’ll continue to need.
      • Becky Kush discussed the BRIDG model. It has gone through all the HL7 ballots and has one more ballot in ISO in late December for FDIS. It will, with luck be ISO/CEN standard by May.
      • CTRR is also in progress notes Becky. Gone through NWIP process and is JIC project in progress at this point.

  12. Future meetings - Austin Kreisler
    • What do we want to do in the future with this meeting. Charlie had suggested in the past that written reports could be submitted and then the time face-to-face be used for other purposes.
      • First timer commented that the agenda was very good for someone just learning about HL7 that might have been familiar with ISO but not with TC 215. The First-Time Attendees session is another good resource, which will be held at Monday lunch.
      • Austin finds silence to be indicative that this format is still acceptable; John suggests that we need a more formal process for getting commitment for those that are here at the meeting to attend and give their presentations. Austin will endeavor to better herd the cats for the future.
